Rural Health Assessment Survey
The Ohio Rural Health Association wants to hear from you! Your feedback will help identify needs and priorities related to maternal health, diabetes care, access to healthcare, healthcare workforce challenges, and community health needs across rural Ohio.
The survey takes approximately 15–20 minutes to complete. Participation is voluntary, responses are confidential, and data will be reported only in aggregate form. Questions that do not apply to you may be marked N/A.
